We are looking for a Home Care Aide to join our team at our current locations in Mesa, Chandler, Tempe, Gilbert, Phoenix, Glendale and Queen Creek!!!!!
You will be directly working with patients, following a one-on-one care Person Centered Plan in an up to 3 patient home. You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus, but not necessary Agency will train. Paid Agency Training and assistance with State required trainings and licenses. Monthly meetings and monthly incentives. Fast, computerized, efficient hiring process.
Responsibilities
Assist patients with daily activities such as hygiene care, appointments completions, Outings into community, Mealtime prep / assistanceCare for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal careMaintain records of patient care, condition, progress, or problems to report and discuss observations with supervisorBuild relationships with patients by interacting, conversing or reading aloud to help keep them mentally healthy and alertGive and document medicationsEngage patients in exercises, dance, movies, board games, video gaming, or other activitiesAssist Behavioral Specialist with hands off behavior techniques (Hands on only by certified staff in severe 911 cases only)Qualifications
