Position Overview

We are seeking an experienced Home Health Occupational Therapist to join our dynamic team. The ideal candidate will be passionate about improving patients' quality of life by providing expert occupational therapy services tailored to individual needs. This role allows for autonomy while working closely with interdisciplinary teams to ensure the highest standards of care.

Responsibilities

  • Conduct comprehensive assessments of patients' physical and functional abilities.
  • Develop and implement personalized treatment plans in accordance with physician recommendations and patient goals.
  • Deliver skilled occupational therapy interventions to improve mobility, strength, balance, and daily functioning.
  • Educate patients and caregivers on therapeutic exercises, safety practices, and injury prevention.
  • Monitor progress, reassess goals, and adjust treatment plans as needed to optimize outcomes.
  • Accurately document assessments, treatments, and patient responses in a timely manner.
  • Collaborate with physicians, nurses, and other healthcare professionals to coordinate care.
  • Ensure compliance with all regulatory standards, including HIPAA and Medicare guidelines.
  • Stay current with best practices and participate in professional development activities.
  • Supervise Certified Occupational Therapy Assistants (COTAs).

    • Qualifications

  • Graduate of an accredited occupational therapy program with a current Virginia license .
  • Minimum of 3 years of clinical experience , preferably in home health or skilled nursing.
  • Strong assessment, treatment planning, and therapeutic intervention skills.
  • Ability to recognize medical emergencies and respond appropriately.
  • Excellent interpersonal and communication skills for patient and team collaboration.
  • Proficiency in conducting comprehensive evaluations and creating evidence-based plans.
  • Independent, organized, and able to manage time effectively in a fast-paced environment.
  • Experience with EMR systems; Kinnser Wellsky or Homecare Homebase preferred.
  • Reliable transportation and a valid driver's license required.
