Summary :

We are seeking a dedicated Licensed Vocational Nurse who is passionate about providing exceptional care to our patients. This role involves understanding and addressing the physical, psychological, social, cultural, and spiritual needs of patients while actively collaborating with our health care team.

Responsibilities :

• Assist the team leader in evaluating patients with simple health issues upon admission and continuously monitor changes in their health status, promptly reporting to the RN or Physician.
• Contribute valuable insights to enhance the plan of care.
• Implement the agreed-upon care plan under the guidance of the RN or Physician, ensuring that the basic needs of patients and their families are met.
• Carefully document all assessments, interventions, and care effectiveness in patients' medical records, reporting findings as necessary.
• Participate actively in patient education initiatives directed by the RN or Physician.
• Support the preparation of patients and families for discharge, under the oversight of the RN or Physician.
• Administer medications as per clinical standards for assigned patients.
• Respond efficiently to emergencies and identify safety hazards to report.
• Exhibit competence in delivering patient care that aligns with the age-specific and developmental requirements of patients.
• Adapt assessment, treatment, and care methods to cater to the unique needs of each patient.
• Uphold the CORE values of Santa Rosa Health Care within your practice.
• Perform other related duties as needed.
