Key Responsibilities 1. Clinical Care & Preventive Management
- Deliver comprehensive primary care services including physical exams, chronic disease management, and preventive screenings.
- Develop and manage treatment plans in alignment with evidence-based guidelines and managed care protocols.
- Focus on proactive care to reduce hospitalizations and improve patient outcomes (e.g., A1c control, hypertension, HEDIS metrics).
2. Managed Care Coordination
Collaborate with care management teams to coordinate services, referrals, and follow-up care.Ensure accurate documentation and coding to support risk adjustment and compliance with managed care requirements.Participate in utilization reviews and quality improvement initiatives.3. Patient Education & Engagement
Educate patients on disease prevention, wellness strategies, and treatment compliance.Build strong relationships through effective communication, empathy, and culturally competent care.4. Team Collaboration
Supervise or work closely with nurse practitioners, physician assistants, and medical assistants.Participate in daily huddles, interdisciplinary team meetings, and care coordination calls as needed.Mentor junior providers or clinical staff (if applicable).5. Compliance & Reporting
Maintain compliance with CMS, HEDIS, and Medicare / Medicaid guidelines.Document all visits and procedures in the EHR system with accuracy and timeliness.Monitor and meet performance goals tied to managed care contracts (e.g., STAR ratings, quality metrics).6. Administrative & Operational Duties
Support quality assurance programs, patient satisfaction efforts, and panel management.Review and sign-off on lab results, diagnostic testing, and refill requests.Participate in audits, peer reviews, and performance evaluations.Bilingual English / Spanish preferred.
Experience with value-based care is a plus
Compensation :
Starting at $230k , with higher pay based on experiencePerformance-based bonuses
