About The Position

Must-Have :

Proficiency in OASIS and HomeCare HomeBase EMR within the last 6 months is required.

Description :

Provides, coordinates, and directs the provision of home nursing care according to physician's orders based on agency policies and procedures.

Applies competent nursing processes in care delivery.

Responsible to the Clinical Manager.

Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Home visits and periodic office meetings required.

Must have reliable transportation and a current driver’s license.

Agency-required liability insurance is compulsory.

Average patient load is 5-8 patients per day.

Contractors are assigned between 5-10 patients daily, totaling roughly 30 points per week.

Types of cases include Start of Care, Wound Care, and Follow-up Visits.

Clinicians must be open to traveling up to 75 miles from the assigned care center.

Mileage is reimbursed at the IRS standard rate.

Documentation must be completed within 24 hours of patient appointments.

There are no guaranteed hours with this contract.

Weekly productivity points expectation is 30, equating to approximately 40 hours per week.

Float opportunities may be available to other nearby clinics if needed.

Home Health Checklist is required for all candidates.

Local candidates are accepted at the same rate.
