Hospice RN Case Manager

Valid drivers license and clean driving record

M-F, and every 3rd weekend rotation 8am to 4 : 30 or 9 : 30 to 5 : 30

Must have 1 year of RN Hospice Case Management experience with Hope Assessment, pain and symptom management.

Caseload expectation is 15-18, 5 points per day.

Brief Unit Description : We see patients in their home (private, LTC, AL) to manage their symptoms while supporting their end-of-life journey. We have IDT once a week.
