The Project Coordinator is under the supervision of the Program Director / Senior Leadership and is the key staff member assisting associates to successfully transition from their previous program or school environment to a community-inclusive lifestyle. The Project Coordinator develops a close working relationship with the associate, his / her family, the Regional Center, and others significant to the associate’s success. The Project Coordinator is immersed in all aspects of the initial Productive and Personally Meaningful Life (PPML) Plans / Person-Centered Planning process, including expertise in the PATH planning model.
The Project Coordinator plays a key role in reaching out to the community to develop the educational, community service, social / recreational, volunteer, and employment opportunities that emerge from the PCP of the associates. Along with the Program Manager and Program Director / Senior Management, the Project Coordinator serves as a part of the leadership group representing the program at parent, school meetings, regional center meetings, and other public meetings.
Principal Areas of Responsibility :
Life Planning / Discovery : Manage all aspects of the referral and intake processes. This includes, but is not limited to : Assisting new associates and their families in adjusting to the new demands / expectations of the program. Assist associates and families with scheduling, transportation, and other logistics to maintain a successful work and / or college day. Work directly with associates and their families and program staff to prepare for and facilitate an individualized person-centered plan. This plan will be conducted after the Discovery process. Work with associate, family, and program staff to implement key components of the PCP. Maintain all necessary documentation regarding associate progress, including semiannual and annual reviews, progress notes, and other relevant correspondence, as applicable.
Program Outreach and Development : Under the supervision of the Program Director / Senior Leadership, the Project Coordinator will develop and sustain positive working partnerships with various community employers, volunteer sites, public education facilities, cultural, social, recreational, and civic organizations. Specific community partnership development activity will be defined and driven by the person-centered plans (PCP) developed for each associate and the need for various options for each new associate entering the program. The Project Coordinator will create awareness among policymakers, community leaders, and education professionals (via meetings, presentations, informational materials) of the need for mainstream opportunities for young adults with multiple disabilities to facilitate partnership building between Tierra del Sol and the community. The Project Coordinator will educate community partners to ensure that program associates are fully valued and included. The Project Coordinator will assist in identifying and overcoming barriers to accessibility and full participation with community partners. The Project Coordinator will assist the Program Director / Senior Leadership in developing the program infrastructure, including training materials, systems, tools, and methodologies. The Project Coordinator will assist in the training of program staff.
Associate Services : Represent the program in outreach activities with Regional Centers and public schools. Participate in outreach and intake for potential new program associates. Lead and / or facilitate the new associate and family orientation process.
This is a 35-40-hour per week position with typical hours of 8 : 00 a.m.-4 : 30 p.m., Monday through Friday. Marketing presentations, associate / family orientations, and intake meetings may require work on evenings or weekends. If necessary, staff will have a 30-minute working lunch when actively supporting associates. A working lunch agreement will be on file. These hours and duties are subject to change.
Qualifications :
AA or Bachelor's degree in Social Services / Rehabilitation or Psychology.
Experience with developmentally disabled individuals is helpful.
Working knowledge of task analysis and positive behavior support techniques.
Bilingual Spanish skills are preferred.
Clear written and verbal communication skills and experience with Word, PowerPoint, and Excel.
Experience in graphic programs.
Must meet all Direct Support Professional qualifications written in the DSP Universal Job Description.
