As an Internal Medicine Physician , you will play a pivotal role in providing exceptional outpatient care, focusing on health maintenance and disease prevention for our senior patients. You’ll lead a dedicated multidisciplinary care team, ensuring that every patient receives personalized attention and the highest quality of care.

Your Role and Responsibilities

As a Primary Care Physician (PCP), you will :

• Evaluate and Treat : Provide comprehensive care to center patients in line with established standards.
• Coordinate Care : Assist in managing patient services, including specialty referrals and hospital coordination.
• Engage with the Team : Actively participate in daily huddles, sharing your medical expertise with the care team.
• Document Effectively : Complete all medical records promptly, collaborating with quality-based coders to enhance coding accuracy.
• Drive Quality : Monitor care quality and outcomes, working closely with leadership to uphold high standards.
• Participate in Growth : Contribute to the development of new services and initiatives within the center.
• Focus on Patients : Dedicate 100% of your time to direct patient care, ensuring a patient-centered approach.
