Population Health Medical Assistant
The Population Health Medical Assistant is responsible for performing a variety of administrative and clinical duties, in order to assist providers with population health activities. This role assists providers and staff in coordinating patient care, scheduling appointments, and entering information in electronic medical records systems, as well as functions as a liaison between patients, coworkers, and providers.

What You Can Expect :
- Prechart encounters in the Electronic Medical Record (EMR) system, review future scheduled patient visits. Contact patients to verify necessary testing for upcoming visit. Update patient medications and allergies. Follow-up on outstanding items in the EMR. Communicate with external healthcare providers to obtain records and scan into chart.
- Review Health Maintenance topics due or overdue with patient. Verify all results are in the EMR. Add pending order for testing as allowed by provider.
- Improve patient outcomes by identifying patient needs for referrals to population health resources including RN, social workers or health coaches.
- Collaborate with primary care providers and staff to coordinate care for patients. Effectively communicate with providers, staff, and clinic resources regarding patient concerns or needs.
- Contact patients throughout the year for focused outreach
- Use designated software to track and document patient progress and outcomes
Qualifications :
High School Diploma / GED requiredCompletion of Medical Assisting program OR 5 years of medical assistant experience requiredCertified Medical Assistant (CMA), Registered Medical Assistant (RMA) Certified Clinical Medical Assistant (CCMA), or National Certified Medical Assistant (NCMA) required3 years of Medical Assisting experience required1 year of Epic experience required1 year of primary care experience preferredBasic Life Support Program (BLS) - American Heart Association requiredTravel is required : Never or Rarely
