LCSW NICU Case Manager

A company is looking for a NICU Case Manager, LCSW (Remote).

Key Responsibilities :

Empower caregivers and families to support optimal wellness and advocate for necessary services

Assess, plan, and implement individualized care strategies for high-risk newborns

Build relationships with caregivers and healthcare providers to address barriers to optimal outcomes

Required Qualifications :

Licensed Social Worker in MD, SC, PA, MI, NY, and / or IN

Minimum of 2 years of experience in Pediatrics / NICU managed care or clinical environment

Valid unrestricted Social Worker license in state of residence; must obtain licensure in other states as needed

5+ years of managed care experience in a case management role for telecommuting positions

Experience with Medicaid populations and understanding of social determinants of health preferred
