We are seeking a highly experienced and dedicated Licensed Physical Therapist Assistant (PTA) to join our Adult Home Health team. This role is crucial in providing high-quality, patient-centered physical therapy services directly within the patient's home environment.   • Current and valid North Carolina Physical Therapist Assistant (PTA) license.
  • Minimum of one (1) year of recent, verified Adult Home Health experience.

Primary Responsibilities

The PTA will work under the direct supervision of a Licensed Physical Therapist (PT) and in collaboration with the patient's attending physician.

Patient Care & Treatment

  • Execute physical therapy treatment plans as prescribed by the PT and ordered by the physician to relieve pain and develop or restore function and independence.
  • Instruct patients and their families in the safe and proper use of assistive devices(canes, walkers, wheelchairs), as well as prosthetic and orthotic devices, as directed by the PT.
  • Observe, accurately record, and report any changes in the patient's condition or reaction to treatment to the supervising PT and clinical staff.
  • Documentation & Communication

  • Maintain timely and accurate patient records, including writing progress notes on the date of the visit.
  • Utilize the electronic medical record system (e.g., McKesson) for incorporation of notes and ensure visits are downloaded / communicated twice daily.
  • Collaborate closely with the Physical Therapist to optimize caseload management and meet established productivity goals.
  • Engage in regular, weekly communication with the supervising PT and daily communication with the interdisciplinary team (RNs, Directors) to ensure coordinated, comprehensive patient care and proper scheduling.
  • Professional Expectations

  • Display a consistently high degree of courtesy, tact, and professionalism in all interactions with patients, families, staff, and visitors.
  • Attend all required staff meetings, rehab meetings, and in-service programs.
  • Maintain compliance with all required hours of in-service training and strictly adhere to the Agencys personnel policies and Compliance Plan.
  • Assist with the orientation of new staff as needed and willingly perform other duties assigned by the supervisor.
