Concord Health Care Services is looking to hire for an In-Home and Community Care Support Individuals that are fun, caring, and outgoing and that are looking for flexibility and a fulfilling relationship with clients.

We are looking for part time availability and potential grow to full time position. We are hiring in Washington, Dakota, Ramsey and surrounded counties (Woodbury, St. Paul, Inver grove Heights, Eagan...). Come join our mission in keeping clients stay safe and comfortable in their own homes and increase their access to community.

Responsibilities will include but not limited to transportation, shopping, errands, grocery shopping, light cleaning around the home, organizing, meal prepping, help with laundry, folding, and light cleaning around the living space and the kitchen area.

Minimum Qualifications

Experience working with individuals with intellectual disabilities, mental illness, and or co-occurring diagnosis.

Experience with verbal de-escalation strategies and working with individuals who exhibit aggressive behaviors.

Must be at least 18 years of age

basic math and computer skills

Must possess and maintain a valid Driver's License, a vehicle and proof of insurance.

Must be able to perform all physical / essential requirements with or without reasonable accommodation(s).

Preferred Qualifications

Experience collecting and summarizing data to document program delivery by observation, charting and measuring.

Job Type : Part Time

Pay : $17.00- $19.00 per hour

Schedule :

• 4 hours shifts
• Day shifts
• Monday - Friday
• Weekend Schedule available
