Job descriptionConduct comprehensive in-home assessments and reassessments of members’ health and functional status. Develop and maintain individualized care plans tailored to each member’s needs. Coordinate and authorize covered services across providers and payers. Communicate and collaborate with members, caregivers, and interdisciplinary teams to ensure optimal outcomes. Serve as a clinical advocate, promoting member well-being and continuity of care. Maintain accurate documentation and compliance with MLTC guidelines and policies. Active NYS RN license & BSN Minimum 3 years RN experience; MLTC or geriatrics experience preferred Salary : $105K–$115K + $20,000 sign-on bonus Hybrid schedule : 4 days remote / 1 day field visits (Bronx office + Manhattan field work) Professional development and growth opportunities Bilingual candidates (Spanish, Chinese, Mandarin) highly encouraged
