A Brief Overview

INSPIRE 360 First Episode Psychosis Case Managers play a critical role in supporting clients through community-based services and coordinated specialty care under the supervision of the program manager. They conduct needs assessments, set case management goals with clients, and facilitate access to community supports and family resources. Additionally, they assist with transitions from inpatient or higher levels of care, including discharge planning.

Case Managers maintain up-to-date knowledge of resources such as benefits, financial assistance, and housing options to provide tailored support to clients and their families. They also act as consultants to clinical teams, promoting client engagement in treatment and program activities while collaborating with multidisciplinary teams to ensure high-quality care delivery.

Through assertive case management, Case Managers meet clients in their homes, schools, workplaces, and communities, fostering active participation in care. Their work emphasizes cultural sensitivity, individualized client-centered care, and helping clients develop life skills, establish healthy daily routines, and pursue meaningful goals to enhance recovery and overall well-being.

Responsibilities

  • Coordinate community resources and family supports.
  • Support clients in building life skills, daily habits, and achieving meaningful recovery goals.
  • Assist clients in navigating benefits, financial assistance, housing options, and other resources.
  • Act as consultants to the clinical team to enhance client engagement.
  • Facilitate discharge planning, including timely implementation of plans for patients with complex needs, in collaboration with interdisciplinary teams.
  • Provide education and consultation to clients, families, and medical personnel regarding case management and discharge planning.
  • Participate in developing treatment plans and provide accurate documentation of sessions.
  • Provide individual and group therapy sessions.
  • Contribute to departmental program planning, goal setting, systems development, and process improvement.
  • Prepare and maintain required records and documentation.
  • Collect, analyze, and report data for utilization, quality improvement, compliance, and other areas.
  • Collaborate with physicians, social workers, nurses, and other team members to assess and plan patient care.
  • Engage in quality improvement activities to identify opportunities for better clinical outcomes and resource utilization.
  • Communicate with third-party payers to ensure reimbursement for services and address queries.
  • Perform utilization reviews to ensure appropriate levels of care and resource use, addressing variances when necessary.
  • Education Qualifications

  • Bachelor's Degree from an accredited college or university.
  • Experience Qualifications

  • Preferred : Three (3) years of progressively responsible and directly related work experience.
  • Minimum : One (1) year of experience providing case management services.
  • Minimum : Two (2) years of experience working with SMI (Serious Mental Illness) and SED (Severe Emotional Disturbance) populations.
  • Experience working with youth and families is required.
  • Required Knowledge, Skills, and Abilities

  • Ability to conduct patient assessments and develop plans of care.
  • Ability to coordinate patient access to appropriate care levels, providers, settings, and services.
  • Proficiency in providing age-appropriate assessments and interventions.
  • Expertise in available treatments, services, and resources for clients.
  • Strong interpersonal skills to resolve conflicts and negotiate positive outcomes.
  • Understanding of federal and state compliance laws and regulations for healthcare.
  • Knowledge of case management models, discharge planning, utilization review, and quality assurance practices.
  • Familiarity with the continuum of care, mental health conditions, and healthcare financial processes.
  • Respect for clients’ beliefs, traditions, and identities.

    Base Pay Scale : Generally starting at $23.88 - $104.39 per hour
