Summary of Essential Functions for the Position :

Committed to Caring , A qualified Licensed Vocational Nurse (LVN) administers one-on-one skilled nursing care to patients on an intermittent basis in their place of residence. The nurse collaborates with physicians, patients and their families, nurses, therapists, social workers, and other members of the patient care team to implement and follow a patient specific care and treatment plan in accordance with the physician orders and under the direction of the Registered Nurse (RN). The nurse will assess, monitor, document and report progress of patients’ health and condition using required clinical notes in the clinical technology within established timelines. Services are furnished in accordance with Agency policies.

Qualifications

Qualifications / Licensure / Certification / Knowledge / Skills / Abilities :

• Active Licensed Vocational Nurse (LVN) license in the state(s) of practice required.
• Minimum of one (1) year experience in nursing, preferred.
• Two (2) years experience, home health preferred.
• Proficiency with EMR systems (e.g., WellSky / Kinnser).
• CPR certified with American Heart Association or American Red Cross required.
• Ability to exercise initiative and independent judgment
• Strong communication skills, both oral and in writing, with excellent interpersonal skills
• Must demonstrate good customer relations skills and a commitment to providing quality service
• May be required to be on call at least once per month
• Valid drivers license, automobile liability insurance and reliable transportation required
• Acceptance and ability to demonstrate support the core values and goals of Agency
• On-call may be required
• 100% travel required
• Requires a valid drivers license and automobile insurance.
