Job Summary
The Wound, Ostomy, and Continence Nurse (RN) is responsible for conducting health assessment interviews, performing focused physical assessments, and developing individualized care plans for patients with wounds, ostomies, and continence-related conditions. This role includes direct patient care, staff education, and collaboration with the healthcare team to ensure best practices in wound care management.
Key Responsibilities
- Conduct focused assessments, develop care plans, and recommend appropriate medications / treatments.
- Assess / document patient response to wound, ostomy, continence, fistula, and complex wound treatment.
- Collaborate with the healthcare team and make referrals as indicated.
- Educate patients, families, and staff on prevention and management of complications.
- Provide staff education programs, orientation, and continuing education.
- Perform risk assessments for pressure injuries and implement preventative measures.
- Participate in Quality Improvement initiatives at both the unit and hospital-wide level.
- Maintain confidentiality of patient data and ensure culturally competent care.
- Manage wound care-related assets such as specialty beds and negative pressure devices.

Education :
Associate’s Degree in Nursing (ASN, ADN) from an accredited program (required)Experience :
Minimum 2 years of Med / Surg experiencePrior Wound and Colostomy experienceLicensure & Certifications (Required) :
Active New York State RN LicensePrimary Source Verification of license and registration (NYSED)AHA BLS CertificationOne of the following wound care certifications :
