Job description

A health services organization in Northern Massachusetts is currently seeking a Social Worker (LCSW) to join their growing team.

About the Opportunity :

  • Schedule : Monday to Friday
  • Hours : 8 : 30am to 5pm

Responsibilities :

  • Assess emotional factors related to terminal illness, as well as environmental resources and obstacles to maintain safety
  • Provide emotional support, short-term crisis intervention, and individual and / or family counseling
  • Maintain contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals
  • Evaluate patient / family response to psychosocial interventions
  • Assess special needs related to cultural diversity, including communication, space, role of family members, and special traditions
  • Perform comprehensive initial psychosocial assessments
  • Assess the patient / family psychosocial status, potential for risk of suicide, and / or abuse or neglect
  • Perform other duties, as needed

    • Qualifications :

  • Master's Degree in Social Work
  • Active Massachusetts Licensed Clinical Social Worker (LCSW) and / or Licensed Independent Clinical Social Worker (LICSW)
  • Solid understanding of Hospice philosophy related to families in crisis and end-of-life issues
  • Strong verbal and written communication skills

    • Desired Skills :

  • 1+ year of related Social Work experience
  • Hospice background
