Ohio Licensed Field RN Case Manager

A company is looking for a Field RN Case Manager.

Key Responsibilities

Manage and coordinate patient care in various settings, ensuring adherence to treatment plans

Conduct assessments and evaluations to determine patient needs and progress

Collaborate with healthcare teams to facilitate optimal patient outcomes

Required Qualifications

Current unrestricted RN state licensure

Preferred Case Management experience and certification (CCM)

At least 3 years of clinical practice experience in relevant settings

Experience in the healthcare or managed care industry

Proficiency in computer skills and navigating multiple systems
