Your Role as a Home Health Physical Therapist

Here’s a snapshot of what you’ll be doing :

Deliver home-based physical therapy to patients recovering mobility, strength, and functionality after illness or injury

Collaborate with a dedicated home health team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to focus on patient care and goals

Observe movements, conduct assessments, document progress, and adjust care plans as necessary

Guide patients through exercises and hands-on therapy to alleviate pain and enhance mobility

Educate patients about their care plans, goals, exercises, and self-care techniques

Support families in understanding the care plan and safely assisting with exercises

What We’re Looking For

A few must-haves for our Home Health Physical Therapists :

Graduation from an accredited Physical Therapist Program and an active PT license in Michigan

A minimum of 1 year of physical therapy experience, preferably in home healthcare within the last 5 years

Current CPR certification

Familiarity with state and federal home health regulations

Strong clinical judgment, excellent interpersonal skills, dexterity, and a compassionate approach
