Job Summary :

We are seeking a compassionate and skilled Physical Therapist (PT) to provide patient-centered care in skilled nursing and post-acute settings. The PT will evaluate and treat residents, develop individualized treatment plans, and collaborate with an interdisciplinary team to ensure coordinated care. Accurate documentation and participation in care conferences and quality improvement initiatives are essential. This role offers flexibility with full-time or travel / contract options.
