Job Description & Requirements Physical Therapist - Home Health - (PT - HH) StartDate : ASAP Available Shifts : 10 D Pay Rate : $1534.08 - $1680.96 Tell us about the unit(s) :

• Number of Beds – approximately 70 patients that are seen by any combination of disciplines
• Number of Staff – 25 (including nursing, rehab, office, and administrative staff)
• Type of staff : Works with other PT’s, PTA’s, OT, RN’s, CNA’s, SW. This service is carried out where the patient calls home : personal home, ALF o Day Shift – Office open 8-4 : 30 Monday-Friday (Nurse on call 24 / 7) o Night Shift - NA
• Patient Ratios – 6 visit equivalents per day ( types of visits are weighted differently on a point system)
• Type of equipment –
• EMR - Cerner Typical hiring profile Skill Set Most recruited for : Home Health Must have : candidates without these skills will not be considered for the role.
• Experience in completing OASIS Assessments
• Home Health experience Preferred or nice to have : candidates with these skills will be considered first.
• Some knowledge of new payment model coming 1-1-20 (PDGM)
• List typical procedures performed on unit(s) :
• Comprehensive assessments / evaluations, treatment and care planning. Oversight of PTA’s.
• Patient population : mainly geriatric; post-acute treatment of THA / TKA patients; treatment of patients with chronic illnesses and multiple comorbidities. Best personality Fit :
• Someone who is easy going, likes to meet people and doesn’t mind visiting people in their own homes. Needs to be accepting of others and friendly. REQUIRED CERTIFICATIONS : BLS / CPR certification. Needs current NC license to practice Physical Therapy Floating Area(s) :
