New York RN Care Manager

A company is looking for a Care Manager (RN) to develop and facilitate care management activities for members with physical needs.

Key Responsibilities

Evaluate member needs and recommend care plans to address barriers to accessing appropriate care

Coordinate with providers and community resources to ensure members receive person-centered care

Monitor member progress and update care plans as necessary to meet their goals

Required Qualifications

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing

2 - 4 years of related experience in care management

NY RN Licensure required; NY residency highly preferred

Registered Nurse - State Licensure and / or Compact State Licensure required
