Are you a Physician Assistant with a passion for rehab medicine, functional outcomes, and geriatric care?
As a Physiatry Physician Assistant , you will work in direct collaboration with a Physiatrist (MD) while supporting rehabilitative and medical management of patients recovering from orthopedic procedures, neurologic events, and other function-limiting conditions. You will play a critical role in optimizing recovery, reducing re-hospitalizations, and supporting patients’ return to independence.

Annual Salary : $110,000 – $130,000 based on experience

Job Requirements
Active and unrestricted Physician Assistant license in Illinois
NCCPA certification required
Experience in PM&R, orthopedics, neurology, geriatrics, internal medicine, or SNF / post-acute preferred
Strong clinical skills in functional assessments, medical management, and rehab-aligned care planning
Comfortable practicing with physician collaboration yet strong autonomy in routine patient management
EMR proficiency and commitment to timely documentation and coordination
Key Responsibilities
In this role, you will serve as a functional medicine specialist within the Skilled Nursing Facility :
Conduct comprehensive medical and functional assessments
Develop, adjust, and co-manage rehab treatment plans in partnership with the supervising Physiatrist
Lead medical follow-ups to monitor progress, optimize pain management, and support safe mobility progression
Participate in interdisciplinary care conferences and team rounds
Collaborate with physical, occupational, and speech therapy teams to track and support rehab milestones
Educate patients and families on diagnoses, recovery timelines, and functional expectations
Complete thorough and compliant EMR documentation in a timely manner
Support safe discharge planning and care transitions
Serve as a trusted consult resource for musculoskeletal and neurologic concerns within the facility
