Palliative Care NP needed in Island County, WA

Your New Role:

• Position: Palliative Care Nurse Practitioner (NP)
• Job Duration: Full Time/Permanent
• Setting: Home Care Hospital Long-Term Care Facility
• Schedule: Variable Monday-Friday, No Call
• Experience:
• 3 years of professional experience in palliative or hospice care required
• Certification in ACHPN preferred, or willingness to obtain within 12-18 months
• Patient Population: Complex patients with palliative care needs, managed in collaboration with a supervising Physician
• Typical Workday: Manage 5-6 patients per day using Meditech EMR system
