Salary : $65 - $70 per Hour DOE

Job Summary :

Aevum Home Health is a fast-growing and locally owned organization with an excellent reputation throughout San Luis Obispo county. We are rapidly growing and seeking a per diem Home Health PT to join our team in the North County area - San Luis Obispo up to Paso Robles .

The Physical Therapist employed through the Organization is responsible for providing Physical Therapy services and for adherence to all conditions outlined in the California Physical Therapy Practice Act.

Work schedule : Regular work schedule is Monday through Friday, 8am-5pm with a choice between 4 or 5 days / week, with a rotating weekend.

Duties & Responsibilities :

• Provides Physical Therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act. This may include, but will not be limited to :

Performing comprehensive assessment and evaluating therapeutic / rehabilitative / functional status, and participation in the development of the total plan of care. Evaluates home environment and safety.
