Job description

JOB PURPOSE :

The Regional Home Care OT will oversee and support the delivery of high-quality occupational therapy services to patients in various settings, such as, but not limited to clinics, home health, or outpatient services, under the direction of Regional Rehab Manager and / or Rehab Director.

This role involves the provision of occupational therapy services across multiple locations within a geographic region.

In addition, this role will have a significant involvement in auditing and quality reviews. The regional Home Care OT will be responsible for making recommendations to the Regional Rehabilitation Manager and Director of Rehabilitation regarding service delivery and documentation of services.

JOB RESPONSIBILITIES :

Direct Participant Care

  • Perform participant evaluations and treatment at the PACE site, outpatient clinic, and / or in the field (participants home, nursing homes, etc.).
  • Documents all pertinent information as required.
  • including but not limited to evaluations, progress notes, care plans, daily statistics, EMR etc.
  • Participates in developing a patient care plan with interdisciplinary team members.
  • Maintain a clean, safe environment and follow safety protocols.
  • Maintains the inventory of supplies and equipment and recommends to the Director of Rehabilitation the purchase of necessary items for the proper functioning of the department.
  • Provides clinical instruction, mentoring, and direction to OTAs, and Students as assigned.
  • Only act within the scope of the individuals authority to practice.
  • Patient Care and Program Development :

Compliance and Quality Assurance :

  • Assist managers in ensuring that all therapy services comply with state and federal regulations, accreditation standards, and company policies.
  • Assist managers in monitoring and reviewing patient records, documentation, and progress reports to ensure accuracy and completeness.
  • Participate in quality improvement initiatives and audits to enhance patient care and operational efficiency.
  • Administrative Responsibilities :

  • Maintain up-to-date records and documentation, ensuring compliance with healthcare standards.
  • Assist in scheduling and coordinating therapy sessions.
  • Participate in administrative and Inter Disciplinary Team meetings and provide reports on team performance, patient outcomes, and operational needs.
  • Included but not limited to; Morning Report, Service Determination Reports, Root Cause Analysis Meetings, Transfer Meetings and Care Planning
  • Other duties as assigned

  • Will provide supervision of Occupational Therapy services in clinic and field as necessary
  • Will provide orientation and clinical guidance to new team members as needed
  • Schedule : 8 : 30 AM 5 : 30 PM

    Weekly Hours : 40

    QUALIFICATIONS : Education :

  • Master's or Doctoral degree in Occupational Therapy (OTD or MOT) from an accredited program.
  • Current, valid New York State Occupational Therapy license.
  • Experience :

  • Minimum of 2 years of experience as an Occupational Therapist preferred
  • Experience in clinical practice across a range of settings preferred (e.g., outpatient, home health, acute care, rehabilitation).
  • Geriatric experience is preferred.
  • Strong knowledge of occupational therapy assessment tools, interventions, and treatment planning.
  • Skills and Abilities :

  • Excellent leadership, communication, and interpersonal skills.
  • Ability to effectively collaborate with a multidisciplinary team.
  • Strong organizational and time-management skills.
  • In-depth knowledge of clinical standards, policies, and best practices.
  • Proficient in electronic health record (EHR) systems and documentation.
  • Ability to problem-solve and make decisions in a dynamic clinical environment.
  • Must have an active drivers license and means of transportation as inter site travel is mandatory
  • Working Conditions :

  • Work outside in various weather conditions in all areas of the community, using private or public means of transportation.
  • Walk to and from patients homes.
  • Potential exposure to health hazards.
  • Work inside in well-lit, heated, and / or air-conditioned offices and / or in varied environmental conditions in the community, which includes participants' homes that the company may not be able to adjust for employee comfort.
  • Physical Requirements :

    Individuals must be able to sustain certain physical requirements essential to the job. This includes, but is not limited to :

  • Standing duration of up to 6 hours a day.
  • Sitting / Stationary positions duration of up to 6-8 hours a day for consecutive hours / periods of time.
  • Lifting up to 50 pounds of equipment, baggage, supplies, and ability to lift patients safely and using OSHA guidelines, etc.
  • Bending / Squatting must be able to safely bend or squat to care for patients, use of medical supplies, etc.
  • Stairs must be able to safely maneuver stairs to access work areas
  • Agility / Fine Motor Skills - Must demonstrate agility and fine motor skills (i.e. proficiency in typing, use of medical supplies, equipment, etc.)
  • Sight / Visual Requirements must be able to visually assess patients, read / enter orders and type / write documentation, etc., with accuracy.
  • Audio Hearing and Motor Skills (language) Requirements must be able to listen attentively and document information from patients, community members, providers, etc., and intake information through audio processing with accuracy. In addition, must be able to speak comfortably and clearly in a manner customers / coworkers can understand.
  • Cognitive Ability Must be able to demonstrate good decision-making, reasonableness, cognitive ability, rational processing, and analysis to satisfy essential functions of the job.
