Position Overview :

As a Home Health Physical Therapist with Connected Health, you will have the opportunity to make a meaningful impact on the lives of patients by providing high-quality, patient-centered therapy in their homes. This position involves evaluating, planning, and implementing individualized treatment plans to improve mobility, strength, and function for patients recovering from injuries, surgeries, or managing chronic conditions.

Key Responsibilities :

• Conduct initial and ongoing evaluations of patients' physical conditions in their home setting.
• Develop and implement individualized treatment plans based on patient goals and medical needs.
• Provide therapeutic interventions to improve strength, balance, coordination, and mobility.
• Educate patients and caregivers on techniques and exercises to support recovery and wellness.
• Collaborate with interdisciplinary teams, including physicians, nurses, and other therapists, to ensure comprehensive patient care.
• Document progress, outcomes, and modifications to treatment plans accurately and in a timely manner.
• Comply with all state and federal regulations, including HIPAA, and company policies and procedures.
