Physical Therapist - Home Health - (PT - HH)

Available Shifts : 8 D

Pay Rate : $1868.33 - $2047.21

2 year(s) of experience required; EMR HomeCare Home Base (HCHB) experience preferred; Home Health experience including OASIS required; must hold active CA PT license at the time of submission

Required Qualifications

Physical Therapist, Home Health

PT-CA(Copy needed)

Certifications : BLS-AHA(Copy needed)

SSN Required

DOB Required

Work History : Should not be more than 30 days of gap in entire work history

References : 2 References in the last 11 months

2 year(s) of experience required; EMR HomeCare Home Base (HCHB) experience preferred; Home Health experience including OASIS required; must hold active CA PT license at the time of submission

M-F 8 : 30-5pm, Rotating weekends (typically 1 weekend every 5 weeks). Mileage is billable per GSA guidelines
