CRITICAL JOB ELEMENTS :
Supervises home care services and may also act as agency director. Responsible for overseeing all agency-provided home care services.
POSITION QUALIFICATIONS :
Must be a licensed NC Registered Nurse with no adverse findings and at least one year of supervisory experience in home care, home health, or other post-acute care settings.
SUMMARY OF JOB FUNCTIONS :
Continuously available for supervision, including on-site when necessary during service hours.
Conducts initial assessments and develops / updates Plans of Care for new clients.
Performs 90-day and annual visits with aides present; manages in-home aides, companions, sitters, and respite services.
Verifies aide and companion / sitter competencies.
Onboards and supervises new in-home aides and companions.
Documents aide, companion, and sitter competency for assigned tasks.
Supervises services as per agency policy.
Supervises companion and sitter services via quarterly client contact and supervisory visits (every six months, annually with companion / sitter present).
May conduct quarterly case reviews, monitoring client condition, progress, and updating Plans of Care.
Supervises in-home aide services via quarterly client residence visits (annually with caregiver present), reviewing client condition and progress.
Documents all supervisory visits, including date, findings, and signature.
Ensures caregiver adherence to job descriptions; promotes customer satisfaction.
Responds to service inquiries.
Develops and maintains in-service training schedules.
Manages employee schedules, assigns shifts, handles callouts, and hiring.
Verifies skills / tasks by return demonstration for hands-on care staff.
Recruits, assigns, supervises, counsels, terminates, interviews, hires staff, and conducts performance evaluations; ensures appropriate staffing levels.
