Is licensed to practice medicine in the state of Arizona.

Is board certified or board eligible in their specialty.

Demonstrates a work history of positive collegial relations with colleagues, support staff, hospital-based providers, administrators and patients.

Demonstrates advanced competency in their specialty.

Schedule / Availability Requirements :

Mon, Tue, Thur, Fri 7 : 30-5 : 30

Setting : Outpatient Clinic

Duties : Family Practice

Days per month : 20

Required Procedures : toenail removals, lesion removals, Pap Smears, Joint injections. (negotiable)

EMR : Cerner

Support Staff : Medical Assistant / Triage RN / Front Office Staff / Clinic Manager

Reason for Coverage : Provider retirement

Travel Preferences (see attachment for specific travel guidelines) :

Preferred Lodging : none. In the event that the Facility preferred hotel is not available, hotel / lodging rates will be reimbursed based on actual expense, up to 1.5 times the current GSA rate for the market.

Air Travel : $750 / round trip cap

Rental Cars : $65 / day all in cap (including taxes and fees) regardless of car class.

Mileage : IRS Standard Rate

Requirements

• Board Certified - REQUIRED
• Active AZ License - REQUIRED
• Fellowship - REQUIRED
• BLS, DEA - REQUIRED
• Availability at time of name clear - REQUIRED
• Malpractice - Clean Preferred
