What You'll Do
The Care Coordination Manager leads a team of Care Guides to ensure high-quality, person-oriented care for members navigating complex health and social needs. This role is responsible for operational oversight, regulatory compliance, team development, training, and strategic alignment with organizational goals.
Key Responsibilities :
Team Leadership & Supervision :
- Supervise and mentor Care Guide Team
- Create high-performance team culture and drive team to achieve all KPIs
- Manage schedules, caseloads, performance monitoring, and evaluations
- Foster a culture of accountability, innovation, and collaboration
- Provide training on workflows / policies, member engagement and communication strategies

Qualifications :
Bachelor's degree in nursing, Social Work, Case Management, or related field, or equivalent experience
Minimum 3-5 years of experience working in healthcare leading and managing teams
Exceptional communication, collaboration, and critical thinking skills
Experience working in healthcare IT systems / care management software / Electronic Health Records
