The Drop-In Center Therapeutic Case Manager (TCM) collaborates with other Housing Department staff to

serve as the entry point into all HUD residential programs. The Therapeutic Case Manager leads clinical efforts to match homeless and unstably housed youth with appropriate services. The Therapeutic Case Manager maintains a caseload of 20 to 30 high needs youth, develops Individualized Case Plans and monitors their progress towards their goals. In addition, the TCM provides brief counseling services to clients, deescalates conflicts and crisis situations and assists clients in other mental health and medication matters. The Therapeutic Case Manager also collaborates with other staff to provide life skills training and other socio-cultural activities to support their successful transition to self-sufficiency.

ESSENTIAL RESPONSIBILITIES

  • Conducts intake assessments and provides community referrals to all new Drop-in center clients.
  • Is available to conduct one-on-one counseling sessions with youth on a weekly or monthly basis.
  • Provides crisis intervention and de-escalation of conflict, as well as reporting Unusual Incidents to DHS.
  • Acts as a liaison with other crisis services providers in the District of Columbia.
  • Enters all case notes and appropriate data into ETO system and HMIS daily.
  • Provides quality control to ensure contract deliverables are met as expected.
  • Assists Program Manager with daily duties of Drop-In Center.
  • Assists clients, as appropriate, in scheduling appointments with other social service providers.
  • Implements the Service Prioritization Decision Assistance Tool (SPDAT) with clients and enters assessment information into the Homeless Management Information System (HMIS).
  • Maintains clients notes up to date, including gathering appropriate documents for case files and requests necessary information from outside agencies.
  • Facilitates group life skills workshops and other youth support groups at least once every week.
  • Provides in depth skills development (life skills, career development, etc.) on an individual basis for youth as needed.
  • Works with community agencies, school leaders, and LAYC / MMYC program staff to develop and foster an effective coordination of services for youth.
  • Provides support to street outreach requirements included but not limited to community canvassing, tabling events and yearly homelessness counts.
  • Represents the LAYC on various city-wide committees dealing with youth homelessness.
  • Collaborates in maintaining working space and organizing youth designated areas.
  • Assists in creating and maintaining safe space for all youth at the Drop-in center.
  • Performs other duties as assigned by the Program Manager.
  • EDUCATION & EXPERIENCE REQUIREMENTS

  • Master’s Degree in Social Work or related field preferred.
  • Minimum three years of significant experience working with at-risk youth.
  • Experience in managing crisis situations.
  • SKILLS & QUALIFICATIONS

  • Knowledge of community and local resources in DC and Maryland.
  • Ability and desire to maintain a flexible schedule (including late nights and weekends).
  • Knowledge of case management, monitoring and reporting.
  • Bilingual or fluent / proficient : English and Spanish required.
  • Complete criminal and child abuse / neglect clearances.
  • Flexible, energetic, positive individual with commitment to youth development in a high quality, respectful environment.
  • Ability to work well and communicate with culturally diverse populations.
  • LAYC / MMYC requires the COVID-19 vaccine of all its employees. You will be asked to provide proof of vaccination at the time of your interview and at your onboarding.
  • PHYSICAL REQUIREMENTS

    This is largely a sedentary position that requires the ability to speak, hear, see, and lift small objects up to 10 lbs. May require the ability to travel locally and / or regionally.
