NP / PA - Outpatient Palliative Care - Wisconsin & upper Michigan

Aspirus Health is seeking palliative care nurse practitioners to join our health system. We currently have outpatient palliative care positions available throughout upper Michigan, and northern and central Wisconsin.

OPPORTUNITY :

• 5-day work week, with scheduling flexibility and no call
• No weekends, no holidays
• 3-6 visits / day, primarily at SNFs, and ASL
• Multidisciplinary team concept
• Strong collaboration with palliative care team, hospitalists, cardiology, oncology, dietitians, and all other specialties as needed
• Potential for virtual visits / telehealth, with excellent IT support
• Highly integrated and optimized EMR allowing for efficient documentation
• Provider must have / obtain a WI & MI license
