Job Summary :

Join our compassionate and collaborative team as an Outreach Specialist, where you will play a vital role in assisting individuals experiencing homelessness to find ongoing support and stable housing. You will have the opportunity to make a meaningful difference in the lives of those in need by connecting them to essential resources and treatment services. We are looking for someone who is empathetic, resourceful, and dedicated to improving the well-being of our community. Your skills in engagement and case management will be crucial in helping individuals navigate their challenges and achieve their goals.

In this role, you will function with the PATH Grant, focusing on both adult and youth outreach efforts. You will engage with individuals who frequently utilize emergency services due to unmet behavioral health needs and provide intensive support to youth and their caregivers in crisis situations.
