    The role you'll contribute :

    \n \n\n \n

    Responsible for facilitating an evidence-based, multidisciplinary, multi-specialty approach to care throughout the care continuum. This position oversees the development and redesign of patient education, evidence-based standards of clinical care, and post-discharge rehabilitation care. This team member monitors program quality and assists with program marketing needs, and gathers, records, and evaluates program data to establish program goals.

  • Participates in marketing efforts through physician visits, educational seminars, and community lectures.

    • \n \n

  • Ensures patients receive appropriate, timely medical and supportive services through a coordinated multidisciplinary approach.

  • Facilitates the integration of evidence-based practices across multidisciplinary and multi-specialty teams throughout the care continuum.

    • \n \n

  • Coordinates the development and implementation of patient education programs based on evidence-based standards.

    • \n \n

  • Oversees the redesign of clinical care standards to ensure they are aligned with the latest evidence-based practices.

    • \n \n

  • Monitors and evaluates program data to establish and achieve program goals.

    • \n \n

  • Develops and tracks program outcomes related to clinical and operational goals in collaboration with the physician and administrative team.

    • \n \n

  • Facilitates problem resolution across departmental, campus, and physician / provider lines.

    • \n \n

  • Conducts data analysis to support efforts related to pay-for-performance reimbursement.

    • \n \n\n \n\n \n

    Qualifications

    \n \n

    The expertise and experiences you'll need to succeed :

  • Diploma Nursing Required

    • \n \n

  • Registered Nurse (RN) [Required]

    • \n \n

  • 3 years of Nursing experience Required

    • \n \n\n \n\n \n

    PREFERRED : \n \n\n \n\n \n

  • Bachelor's of Nursing Preferred

    • \n \n

  • Master's of Nursing Preferred

    • \n \n

  • Bariatric experience [Preferred]

    • \n \n\n \n\n \n
