Position Details : Location :

Reliant Medical Group, a Part of Optum : 900 Union St. Westborough, MA

Schedule :

4.5 days / week (Half-day TBD). Office hours are Monday - Friday; 8 : 00 am-5 : 30 pm. No evenings, weekends, or major holidays

Specialty :

Adult Internal Medicine

Primary Responsibilities :

Determines the appropriate action to be taken involving decision making according to standard criteria

Communicates with providers and patients systematically via EMR or telephonically

Maintains tracking of patient lists, outreach and follow up

Offers clinician and patient support and / or education on workflows and protocols in the identified registry

Works with assigned supervisor / manager and director to develop, implement, and monitor procedures that support organizational goals and business objectives related to the Registry program

Required Qualifications :

Graduate of an accredited School of Nursing

Currently licensed as an RN in the State of Massachusetts

Basic Life Support (BLS) / CPR Certification

Preferred Qualifications :

1+ years of hands-on clinical experience

Knowledge of EMR's such as Epic

Physical Requirement :

Physical health sufficient to meet the ergonomic standards and demands of the position

Unless certification, licensure or registration is required, an equivalent combination of education and experience, which provides proficiency in the areas of responsibility listed in this description, may be substituted for the above requirements.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $28.27 to $50.48 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
