Position Summary : Provides overall leadership, direction, and strategic planning for Case Management services to promote and ensure the advancement of coordinated care across the care continuum. In coordination with leadership, ensures that case management and care coordination services effectively and efficiently meet the needs of all internal and external customers and are compliant with regulatory groups.

The starting salary range for this position is $110,000 - $145,000
