Social Worker

Join our team as a dedicated Social Worker devoted to improving the lives of individuals in need. In this role, you will play a vital part in offering essential support and interventions to enhance the well-being and social integration of our clients. We are looking for a caring individual who is enthusiastic about assisting others and creating a meaningful impact within the community.

Our organization is an Adult Day Health Center that offers care services to seniors and adults facing mental challenges or illnesses. The Social Worker will deliver social services aimed at helping participants and their families resolve social issues stemming from the participant's disability.

Social services shall consist of at least the following:

(1) Initial and continuing assessment of the participant by the social worker in the formation of an individual plan of care by the multidisciplinary team.

(2) Maintaining signed progress records in the participant's health record at least quarterly.

(3) Assumption of responsibility for implementation and coordination of the discharge plan.

(4) Providing referral to outside resources and assisting with counseling when applicable.

(5) Providing problem-oriented discussion groups among participants.

(6) Serving as a liaison with the participant's family and home.

Qualification: Must have bachelor's degree in social work

Work hours: Monday - Friday, 7:30 am - 2:00 pm
