Job descriptionGather and maintain patient information and records, including social and medical history obtained from patients, relatives, and other professionals. Prescribe and administer psychotherapeutic treatments / medications to treat behavioral health disorders and / or substance use disorders at assigned program / sites. Advise and inform guardians, relatives, and significant others of the patient’s conditions and treatment. Examine or conduct laboratory or diagnostic tests on patients, as necessary Attend Quarterly Prescriber Meetings and any other assigned meetings. Adherence to all applicable evidence-based practice models. Demonstrate a working knowledge of diagnostic criteria sets for substance use and / or behavioral health disorders. Orally communicate information effectively and accurately. Assure program operates within the constraints of the agency’s certification (CARF, DMH) standards and strategic plan related to clinical care. Abide by program and agency policies and procedures. Other job duties and special projects as assigned.

Job Purpose
Under clinical supervision of a physician, the Nurse Practitioner provides primary and preventive healthcare services to patients in a rural health clinic, diagnosing and treating acute and chronic conditions, promoting wellness, and improving access to quality care.
Qualifications
- Master’s Degree in Nursing with appropriate license to practice.
Family Nurse Practitioner Certification required.
