Under the administrative supervision of the Director of Transgender Health Programs, the Linkage to Care Specialist is responsible for conducting required screenings as part of the regular course of general care, conduct a service needs assessment, and link the target population, to the appropriate services including a warm handoff to the PRIME and Behavioral Health programs. This position will also conduct assessments and program intakes as required to meet program objectives and support the team to achieve program goals.

Qualifications :

Education & Experience

  • A minimum of 2 years’ experience working with transgender individuals in a social service agency or health center setting.
  • High School diploma or equivalent, Bachelors preferred.
  • Ability to work effectively with a multi-disciplinary team.
  • Excellent facilitation, written and verbal communication skills.
  • Transgender-affirming and knowledgeable about the population of focus.
  • Comfortable initiating internal / external outreach and program coordination activities.
  • Strong interpersonal and communication skills.
  • Excellent time management and organizational skills.
  • Ability to multi-task and work effectively in a time-sensitive environment.
  • Demonstrate ability to anticipate needs and solve problems, prioritize conflicting demands, follow up on tasks and meet deadlines with minimal supervision. Excellent word processing skills with MS Word, Excel, Internet Explorer and Outlook; ability to create programmatic materials.
  • Responsibilities :

  • Provide PrEP, PEP, STI and other risk-reduction education to patients at risk for HIV.
  • Provide medication adherence education.
  • Act as main point of contact for patients seeking to make initial appointments for HIV prevention services or HIV prevention services.
  • Act as liaison between HIV Specialist and patients.
  • Work with interdisciplinary team to provide comprehensive care to people at risk for HIV / AIDS.
  • Provide follow up calls and appointment reminders for patients with HIV prevention medical visits.
  • Coordinate medication refill assistance, including, but not limited to, patient assistance program applications, liaise between patient and specialty pharmacies, contact HIV specialist for patient medication refills, etc.
  • Other duties as assigned. Link patients to appropriate internal and external services as necessary.
  • Screen patients for appropriate benefits and refer to benefits counselor if necessary.
  • Provide HIV prevention education to patients in need of psychosocial and / or socio- economic support.
  • Conduct referrals as needed based on the assessment in program intake.
  • Conduct basic needs assessment.
  • Assess and reassess patients’ medical adherence and determine patient’s need for biomedical interventions.
  • Discuss other HIV prevention options.
  • Assist with housing placement as applicable.
  • Compose correspondence.
  • Assist with collecting information for and completing program reports.
  • Create and maintain program files and records.
  • Attend program related meetings for or in place of Program Director.
  • Perform other duties as directed by Transgender Health Program Director.
