Job Summary
The Behavioral Health Care Manager (ACSW) supports CalAMS ECM program for patients with behavioral health needs by providing care coordination, brief therapeutic interventions, and linkage to community resources. This role helps integrate behavioral health into the primary
care setting and promotes whole-person care through proactive outreach, engagement, and teamwork. It is responsible for being a liaison between the Patient, Primary Care Providers, and Behavioral Health Clinicians.

Essential Duties / Responsibilities
- Manage a caseload of ECM members with behavioral health conditions, including those with Serious Mental Illness (SMI) and / or substance use disorder (SUD), using brief, structured interventions within ACSW scope under supervision.
- Assist with crisis screenings and facilitate warm hand-offs to local crisis or emergency resources as appropriate.
- Provide brief, structured behavioral health interventions (e.g., motivational interviewing, behavioral activation, coping skills training) within ACSW scope under supervision.
- Provide educational support and outreach to members regarding behavioral health conditions and treatment options.
- Assist members in following through care plans and wellness goals through phone and in person engagement.
- Complete Health Assessments and integrate findings into a Shared Care Plan.
- Coordinate with internal and external health partners, including PCP offices, specialists, and ancillary services, to address behavioral health and medical access needs
- Communicate care plans with members, caregivers, and care teams to promote shared understanding and accountability.
- Participate in multidisciplinary team meetings and case reviews to discuss member progress and care updates.
- Build and maintain collaborative relationships with members, providers, and care team members using evidence-based communication strategies (e.g., Motivational Interviewing).
- Promote engagement and self-management by empowering members to participate in their care.
- Model the highest ethical behavior in relationships with co-workers, supervisors, Members, Providers, and colleagues in the community
- Provide formal and informal training and support for team members on behavioral health conditions, including treatments and evidence-base for treatment
- In conjunction with the LCSW and / or ECM Operations Manager, the Behavioral Health Care Manager is responsible for providing consultation for the non-licensed members of the team when discussing tasks of a clinical nature
- Ensure documentation is accurate and in compliance with regulatory requirements and accreditation standards

A master's degree from a Council of Social Work Education (CSWE)-accredited programMust be registered with the Board of Behavioral Sciences (BBS) as an ACSWMust provide proof of completing and passing the California Law and Ethics ExamPrevious experience in the medical field and / or in the behavioral health / mental health field, preferredWorking Conditions / Physical Requirements
This position is on-site at a DAP Health clinic location with the possibility of hybridThis job operates in an office setting and requires frequent times of sitting, standing, repetitive motion and talkingAbility to lift / move up to 40 pounds, move from place to place, and stand for long periods of timeAbility to travel to different locations as needed to support program and patient care needs.Requires current and valid driver's license and current personal auto insuranceAble to travel to DAP Health locations throughout San Diego and the Coachella Valley
