RN Clinical Navigator - Cancer Program

The Care Coordinator is responsible and accountable for ensuring high quality patient care that is coordinated, efficient, efficacious and supports desired clinical and financial outcomes for the Breast,Cervical and Colorectal Cancer Program. Accountable for daily program operations, collaborating closely with the health care team. Assists in developing systems for program; provides data for State reporting; teaches individual patients on-site and in the community; and follows up on patient care activities. Ability to quickly process complex problems, identify key components and develop plan for resolution. Effective interactions with all healthcare team members and the ability to elicit their cooperation. Ability to provide clinical / financial education to colleagues, patients and families.

Responsibilities :

• 1. As part of the interdisciplinary health care team, coordinates and ensures the implementation of the plan of care, utilizing the principles of case management.
• 1.1 Establishes a system for coordinating the care of a patient throughout the continuum of care and linkage to resources through case management services.
• 2. Optimizes the efficiency of hospital systems which impact quality care and case management services.
• 2.1 Identifies and monitors compliance with documenting variances from established parameters in the clinical pathway or treatment plan related to hospital systems.
• 3. Utilizes information obtained from various resources available to :
• 3.1 Ensures that patients meet eligibility requirements and clinical needs for program services within established policies and procedures.
• 4. Ensure that the appropriate outside agencies are contacted and necessary referrals are initiated and followed through.
• 4.1 Links patients with the appropriate institutional or community resources, advocating on their behalf for scarce resources, and developing new resources where gaps exist in the service continuum.
• 5. Along with other members of the health care team, acts as a patient advocate.
• 5.1 Exhibits awareness of ethical / legal issues concerning patient care and strives to manage situations to reduce risk.

Qualifications : EDUCATION

Minimum of a Bachelor's degree in clinically related field. RN required. Strong interpersonal skills and computer skills. Accountable for continuity of care for breast and cervical cancer program patients. Ability to work independently

EXPERIENCE

Minimum of five (5) years of relevant clinical experience (with at least two (2) years in specialty area of placement).

SPECIAL SKILLS

Clinical credibility with physicians, nurses, and other members of the health care team. Interpersonal skills to facilitate negotiation for scarce resources, facilitate changes in systems and / or practices and to broker with outside agencies. Excellent communication skills with the ability to communicate effectively with multiple disciplines and skill levels. Ability to quickly process complex problems, identify key components, and develop plans for resolution. Effective interactions with all health care team members and the ability to elicit their cooperation. Demonstrated skills in clinical management of specific patient populations including working with and providing support and case management. Ability to provide clinical / financial education to colleagues, patients, and families.

Additional Information :

Combination of on site and remote work, flexibility with schedule.

This is a grant funded position. It is a part time clinical position accountable for ensuring high quality patient care that is coordinated, efficient, efficacious and supports desired clinical and financial outcomes for the Breast ,Cervical and Colorectal Cancer Program., Coordinate abnormal findings in women this program. Accountable for continuity of care for breast, cervical and colorectal cancer program patients. Ability to work independently.

Experience : Minimum of 3-5 years of OB / GYN experience with multicultural population and the Greater New Haven community. Experience working with Breast, Cervical and Colorectal prevention programs highly desirable.
