Care Management Support Coordinator III Job Details :

Schedule :  Monday - Friday 8 : 00am - 5 : 00 pm, overtime based on business needs.

Location :  Must Reside in Texas.

Temporary to hire

Starting Pay Rate :  $27.00 / hour + (based on experience)

Care Management Support Coordinator III Responsibilities :

• Conduct outbound calls to members to assist with care plan follow-up, scheduling, and education on available resources.
• Serve as the front-line support for members, providers, and internal staff to resolve inquiries and ensure a positive service experience.
• Support care coordination activities, including sending welcome letters, correspondence, and educational materials.
• Document and maintain accurate member records in accordance with established policies and regulatory requirements.
• Works with care management team on escalating request, inquiries to management and support training of new hires.
• Perform additional administrative duties as assigned

Education Requirements :

High school diploma or equivalent, required.
