Home Health Physical Therapist (PT)

About the Role :

Medadventures is seeking an experienced and compassionate Physical Therapist to provide one-on-one care in a home health setting. This role is ideal for a clinician who values flexibility, autonomy, and meaningful patient relationships.

Responsibilities :

  • Evaluate, plan, and implement individualized treatment programs for patients in their homes.
  • Collaborate with the interdisciplinary care team to ensure continuity of care.
  • Document all visits and progress notes in compliance with home health regulations.
  • Educate patients and caregivers on safe mobility, exercise, and self-care techniques.

Qualifications :

  • Active state Physical Therapy license.
  • Minimum of one year of clinical experience (home health experience preferred).
  • Strong communication and time-management skills.
  • Reliable transportation and valid driver's license.
