JOB PURPOSE / SUMMARY

Summary of role of team :

• By virtue of the persons served, the Health Link Program and Intensive Long-Term Support Programs represent a place where the Center's vision is lived out on a day-to-day basis.
• The severe and persistently mental ill (SPMI) population truly embodies the poorest of the poor and sickest of the sick.
• Therefore, it is the philosophy of the nursing position to live up to the Center's vision by reaching out to the SPMI, regardless of what situation they are in or where they are at in their recovery and open a door of stability through mental health treatment.
• Services provided are team-based and teams are designed to effective coordinate health care services for clients enrolled in Health Link or ILS programming.
• Nursing services are directed toward recovery and self-management of mental illness, provide the consumer / family an opportunity to improve their overall quality of life, provide training in community living skills, and increase socialization through the provision of daily leisure and psycho-educational activities.
• Cultural and ethnic factors are taken into consideration in the delivery of these services.
• Services are not time-limited but designed according to the needs of the individual being served.
• The ILS program is staffed with a minimum of one staff member for every eight service recipients 24 hours per day / 7 days per week / 365 days per year.
• The ILS program provides service to adults who are age 18 and over who have been discharged from one of the state RMHI's or one of the state contracted providers for inpatient psychiatric services.
• The THL caseload may consist of children, adolescents, or adults who meet the criteria for enrollment based on diagnosis, crisis intervention, and / or medical necessity.
