We are currently seeking a Psychiatric Nurse Practitioner (PMHNP) who is passionate about geriatric psychiatry and enjoys working collaboratively within a supportive healthcare environment.
Position Summary
The PMHNP will provide comprehensive psychiatric care to elderly patients, focusing on accurate diagnosis, effective treatment planning, and ongoing management of mental health conditions. This role is ideal for clinicians who value autonomy while also thriving in a team-based care model.
Requirements
Key Responsibilities
Conduct psychiatric assessments and develop individualized care plans for patients in skilled nursing and assisted living settings.
Prescribe, monitor, and adjust psychiatric medications as needed to ensure optimal outcomes.
Deliver therapeutic interventions, including supportive counseling and brief psychotherapy.
Collaborate closely with physicians, psychologists, nurses, and social workers to coordinate holistic care.
Educate patients and families on diagnoses, treatment options, and medication adherence.
Maintain timely, accurate documentation in the electronic health record (EHR)
Qualifications
Active New York State Nurse Practitioner License with a Psychiatric–Mental Health (PMHNP) certification.
Geriatric or long-term care settings is a plus
Strong understanding of psychopharmacology and current treatment guidelines.
Excellent communication, clinical reasoning, and organizational skills.
Compassionate, patient-focused approach to mental health care.
Ability to work independently while maintaining collaboration with the interdisciplinary team.
Proficiency with EHR systems and general computer use.
