Position Overview

We’re seeking a dedicated and empathetic Nurse Practitioner to join our team and provide care to residents at our Skilled Nursing Facility in Hobe Sound, FL.

This role is ideal for a provider who thrives in a collaborative environment and is committed to making a meaningful impact on older adults’ health and well-being.

• Schedule : Full-time preferred, but open to 3–4 days per week depending on availability
• On-site Presence : Must be at the facility during scheduled days (no remote rounding)
• Call : Rotating call schedule, approximately 1 week per month
• Teamwork : Collaborate closely with other NPs, nurses, and medical assistants
• EHR : Documentation in Elation all notes to be completed same day, orders by close of business
