Physical Therapist - Rehabilitation - (PT - Rehab)

Available Shifts : 8 D

Pay Rate : $1664.35 - $1823.70

Shift : M-F 8-4 : 30 rotating weekends

Scrub Color / Attire - Grey scrub top and black scrub bottoms

No floating

Weekend coverage needed - Weekend Requirements : 1-2 weekends a month

On Call Requirements None

EMR – Meditech

Type of equipment – Bioness; FES; Biodex

Patient Ratios – 4-5 / 1

Number of Beds - 15

Utah Physical Therapy License; BLS

Positive, upbeat, encouraging; engaging; compassionate; motivating

Physical therapy evaluations with varying diagnoses

75-90 minutes physical therapy treatment plans; 25-30% of our patients are stroke / neuro

Team player with all disciplines-PT / OT / ST / SW / RN / MD / CRS / PPSC

Developing CARE scoring with all patients for quality / reimbursement purposes

Number of Staff – 3 FT PT; 2 PRN PT; 4 FT OT; 2 PT OT; 3 PRN OT; 1 PRN COTA; 1 FT ST; 1 PRN ST

Dept : IP Rehab Physical Therapy

Required Qualifications

Physical Therapist, Rehab

Experience : 2 years

PT(Compact Accepted)

Certifications : BLS-AHA

SSN Required

DOB Required

Work History : Should not be more than 90 days of gap in last 1 year

Good organizational and Communication skills working with the many disciplines
