Role Overview
An Assertive Community Treatment (ACT) team provides a community-based group of medical, behavioral health, and rehabilitation professionals who use a team approach to meet the needs of individuals with severe and persistent mental illness. This service includes substance use services and structured rehabilitative interventions intended to increase and restore an individual's ability to live successfully in the community. The Substance Abuse Professional will assess and track stages of change readiness and treatment, ensuring that team treatment approaches are consistent with an individual's readiness plan.
Job Duties
Use cognitive behavioral approaches and relapse prevention to work with individuals in later stages of change readiness.Facilitate the Person Centered Planning process for individuals assigned to the specialist.Apply clinical processes to achieve positive outcomes, including evaluation, assessment, service, treatment planning, intervention techniques, aftercare, and transition planning.Remain available face-to-face or by telephone, 24 hours a day, 7 days a week for crisis situations and first responder duties at the consumer or family's location.Basic Requirements
Computer proficiency with Microsoft 365 (Word, Excel, Outlook) and other basic programs.Valid driver's license, auto insurance coverage, and reliable transportation.Pass criminal background check and other regulatory checks.Provide proof of education.Education and Experience Required
LCAS : Bachelor's degree in Human Services, Social Work, Special Education, or a closely related field.CADC : High School education.One (1) year of professional experience with the adult mental health population.Benefits
