The Community Health Worker position is a key position in carrying out daily activities and establishing connection with program participants at the Sobering Center.  The position is central to monitoring and maintaining organization, structure, and safety for all staff, clients and community members arriving and or residing at the Sobering Center.

In this role, you’ll make a difference in the lives of people with substance abuse challenges, and often with co-occurring conditions. You’ll use and grow your skills in compassionate care, crisis intervention and de-escalation, in a supportive and inclusive work environment. You’ll have the opportunity provide our community with substance abuse treatment to help people who are just discovering possibilities in their journey, going from a place of hopelessness to a place where they are valued and have the potential for a better life.

PRIMARY ACCOUNTABILITIES & RESPONSIBILITIES

  • Monitor and observe Sobering Center client behavior and activity to ensure clients are safe, accounted for and stable.
  • Must work collaboratively as part of a multi-disciplinary team at the Sobering Center
  • Create and maintain a safe, welcoming environment for all clients.
  • Maintain facility cleanliness and organization daily
  • Provide help with transportation and emergency response
  • Responsibilities :

    1. Monitor and observe Sobering Center client behavior and activity to ensure clients are safe, accounted for and stable.

  • Conducts property safety checks on client incoming property
  • Monitor daily bed roster and census to ensure client accountability with program requirements.
  • Assist with meal preparation and meal distribution daily.
  • Run reports from EPIC, Medi-Cal and HMIS (MSRS only)
  • 2. Must work collaboratively as part of a multi-disciplinary team at the Sobering Center

  • Communicate daily needs or observational concerns or impression to Health and or SUD staff.
  • Participate in daily shift change activity any concerns
  • 3. Create and maintain a safe, welcoming environment for all clients.

  • De-escalate emotionally unstable client behavior
  • Provide orientation and overview for incoming clients
  • Attends to clients' needs while maintaining daily activity schedule.
  • 4. Maintain facility cleanliness and organization daily.

  • Monitoring cleanliness and performing needed cleanup duties.
  • Preparing bed rolls and basic hygienic items to all incoming clients
  • Ensure laundry details are completed
  • Replenish supplies as directed by Program Manager
  • 5. Provide transportation and emergency response assistance.

  • Transport clients to local facilities when needed (Sobering Center only)
  • Respond to emergency needs as directed by Health Center staff and / or Program Manager
  • Assist SUD or Health Center staff with referral and or transfer (Sobering Center only) to continuing care.
  • 6. Community Health Worker Responsibilities :

  • Cultural Mediation Among Individuals, Communities, and Health & Social Service Systems.
  • Providing Culturally Appropriate Health Education and Information
  • Providing Coaching and Social Support, Advocacy, Cultural Humility and Service Coordination
  • Care Coordination, Case Management, and System Navigation

  • Must be able to use standard janitorial and cleaning equipment.

  • Ability to interface with staff members, community members, clients, family members and other visitors and or professionals.
  • Ability to follow and provide written and verbal directions
  • Ability to recognize stress factors and creative ways to reduce them.
  • Ability to control emotions during times of conflict and high stress
  • Needs to be able to work in stressful situations where information is needed in a brief period.
  • Ability to function as a team member and cooperate with other staff and
  • agencies

  • Ability to work with diverse populations, cultures and people with co- occurring disorders, disabilities, and complex trauma
  • Ability to accurately document verbal and written information received
  • Ability to maintain a welcoming and positive attitude while working.
  • Ability to be flexible to work with organizational and or program changes
  • Ability to respond to crises promptly
